What are piles?

Everyone has soft cushions of blood vessels inside the anal canal. They help control the passage of stool. When these cushions become swollen, stretched or start to slip down, they are called piles or haemorrhoids (बवासीर).

Piles can be internal (inside the back passage, usually not painful) or external (under the skin around the anus, can be painful, especially if a clot forms). Doctors grade internal piles by how far they come out:

GradeWhat happensUsual first approach
Grade 1May bleed, but do not come outDiet, fluids, medicines
Grade 2Come out when straining, go back on their ownDiet and medicines; banding or laser if symptoms continue
Grade 3Come out and need to be pushed back by handBanding, laser or surgery, depending on size
Grade 4Stay outside and cannot be pushed backUsually surgery; laser in selected cases

This table is a general guide. Your surgeon will suggest what suits your piles after examining you.

Symptoms of piles

  • Bright red blood on toilet paper, in the pan or on the stool, usually without pain
  • A soft lump that comes out of the anus while passing stool
  • Itching, irritation or a feeling of wetness around the anus
  • A feeling that the bowel has not emptied fully
  • Pain or a hard, tender lump near the anus: this can happen when a clot forms in an external pile
  • Mucus (slimy) discharge that may stain underwear

Sharp, cutting pain while passing stool is more typical of an anal fissure than piles. A swelling that keeps leaking pus may be a fistula. It is common to have more than one problem at the same time, so an examination matters.

Causes and risk factors

Anything that increases pressure in the lower rectum for a long time can lead to piles. Common reasons include:

  • Long-term constipation and straining to pass stool
  • A low-fibre diet (too little fruit, vegetables, whole grains, dal) and not drinking enough water
  • Sitting on the toilet for a long time, for example while using a phone
  • Pregnancy and the months after delivery
  • Being overweight, and lifting heavy weights regularly
  • Long-standing diarrhoea
  • Age: the supporting tissues become weaker over time
  • Jobs with long hours of sitting or standing, such as driving or shop work

When to see a doctor

See a doctor if you notice bleeding, if a lump keeps coming out, or if home care has not helped within one to two weeks. Get medical help urgently if you have:

  • Heavy bleeding, or bleeding that does not stop
  • Dark red, maroon or black stools
  • Dizziness, weakness or a pale look (signs of blood loss)
  • Severe pain, fever, or a swelling that is getting bigger or hot
  • A lump that cannot be pushed back and is turning dark
  • Bleeding along with weight loss, loss of appetite or a change in bowel habits, especially if you are over 40

Bleeding is not always piles. Please do not treat rectal bleeding at home for weeks without an examination. In an emergency, call 93196 62944. The hospital is open 24×7.

How piles are diagnosed at Samrijo

Diagnosis is usually simple and is done in a private consultation room.

  • Talking with you: your symptoms, bowel habits, diet, medicines and family history.
  • Examination: a gentle look at the area and a finger examination with a gloved, lubricated finger. It takes a minute or two and is usually not painful.
  • Proctoscopy: a short, lubricated tube lets the surgeon see inside the anal canal. This helps grade the piles and check for other problems.
  • Blood tests at the hospital laboratory if there has been a lot of bleeding (to check for anaemia) or before a procedure.
  • Colonoscopy may be advised if the bleeding pattern, your age or other symptoms suggest a cause higher up in the bowel.

Treatment options

Treatment goes step by step, from the gentlest to the most involved. Your surgeon will explain which options suit the grade and type of your piles, and why.

1. Diet, lifestyle and medicines

Grade 1–2

More fibre (fruit, vegetables, whole grains, isabgol if advised), 8–10 glasses of water a day, stool softeners, warm sitz baths and creams for comfort. Not straining, and not sitting on the toilet for more than a few minutes.

Good for

  • No procedure, no anaesthesia
  • Helps prevent future piles too

Keep in mind

  • Needs a few weeks and daily discipline
  • May not be enough for larger piles

2. Rubber band ligation (banding)

Grade 1–3 internal

A tiny rubber band is placed at the base of an internal pile in the OPD. It cuts off the blood supply, and the pile shrinks and falls off in about a week.

Good for

  • Quick OPD procedure, usually without anaesthesia
  • Back to routine the same or next day

Keep in mind

  • May need more than one session
  • A dull ache for a day or two; some bleeding when the band falls off
  • Not for external piles

3. Laser haemorrhoidoplasty (laser piles treatment)

Grade 2–3, selected grade 4

Under anaesthesia, a very thin laser fibre is placed into the pile. Controlled laser energy seals the blood vessels feeding it, so the pile shrinks from the inside over the following weeks, usually without a large external cut. Samrijo uses a diode laser for proctology procedures.

Good for

  • Many patients have less pain afterwards than with conventional surgery
  • Less bleeding; often day-care or a one-night stay
  • Faster return to routine for many people

Keep in mind

  • Not suitable for every pile, such as very large or mainly external piles
  • Piles can come back, especially if constipation continues
  • Long-term data is more limited than for conventional surgery

4. Surgery: haemorrhoidectomy or stapler

Large grade 3–4, external

Haemorrhoidectomy removes the piles with a precise surgical cut. Stapled haemorrhoidopexy lifts prolapsing piles back into place with a special stapling device.

Good for

  • Haemorrhoidectomy has the lowest chance of piles coming back
  • Suits large, external or complicated piles

Keep in mind

  • More pain after surgery and a longer recovery (often 2–4 weeks)
  • Stapler is not suitable for external piles

What to expect with laser treatment

Before

A consultation and examination, blood tests, and a check-up by the anaesthetist. Tell the team about all your medicines, especially blood thinners, diabetes or blood pressure tablets. You will usually be asked not to eat for about six hours before the procedure.

On the day

You are given anaesthesia (the type is decided by the anaesthetist), so you do not feel the procedure. The laser procedure itself is often completed in under an hour.

Right after

You rest in the recovery area for a few hours. Many patients go home the same day or the next morning, as the surgeon advises. Some pain, swelling and a little bleeding with the first few bowel movements are common and are managed with tablets and sitz baths.

Recovery at home

Eat a high-fibre diet, drink plenty of water, take stool softeners as prescribed and avoid straining. Light walking is encouraged. Desk work is often possible within a few days; heavy lifting may need one to two weeks. Recovery time varies from person to person.

Follow-up

A check-up visit, usually within one to two weeks, to make sure you are healing well. Call the hospital any time if you have heavy bleeding, fever or pain that is getting worse.

Frequently asked questions

Is laser piles treatment painless?

No treatment can honestly promise zero pain. Laser treatment is done under anaesthesia, so you should not feel pain during the procedure. Afterwards, many patients have less pain than after conventional surgery, and it is usually managed with tablets and warm sitz baths. How much discomfort you feel depends on the size of the piles and on your own body.

How long does recovery take after laser piles treatment?

Many patients go home the same day or the next day. People with desk jobs often return to work within a few days, while heavy physical work may need one to two weeks. Your surgeon will give you a timeline based on your piles and your work.

Can piles come back after laser treatment?

Yes, piles can come back after any treatment, including laser and conventional surgery. The risk is higher if constipation and straining continue. Eating enough fibre, drinking enough water, not sitting on the toilet for long and following up with your surgeon all help lower the chance of recurrence.

Can piles go away without surgery?

Often, yes. Small (early-grade) piles frequently settle with a high-fibre diet, enough water, stool softeners, warm sitz baths and medicines. Procedures are usually advised when piles keep bleeding, keep coming out, or do not improve with these measures.

Is bleeding from the back passage always due to piles?

No. Piles are a common cause, but bleeding can also come from a fissure, polyps, inflammation or, less commonly, cancer of the bowel. That is why bleeding should always be checked by a doctor, especially if you are over 40, have lost weight, feel weak, or notice a change in your bowel habits.

How much does laser piles treatment cost in Delhi?

The cost depends on the grade of the piles, the technique chosen, the type of anaesthesia, the length of stay, tests needed and your room category. Please call 93196 62944 for an estimate after a consultation.
