Thyroid swellings explained
The thyroid is a butterfly-shaped gland in the front of the neck, below the Adam's apple. It makes hormones that control metabolism. Swellings of the thyroid include:
- Goitre: a general enlargement of the gland, which may be smooth or contain several nodules (multinodular goitre)
- Thyroid nodule: a lump within the gland; very common, and most are benign
- Overactive thyroid (hyperthyroidism, e.g. Graves' disease or a "hot" nodule), which may cause swelling
- Thyroid cancer: less common, usually slow-growing, and often very treatable
Thyroid swellings are common in North India, and very often found by chance on an ultrasound of the neck or during a check-up. Finding a nodule is not a reason to panic: the job of the tests is to separate the large majority that simply need watching from the few that need treatment.
Symptoms
- A visible or felt swelling in the front of the neck that moves up when you swallow
- A feeling of pressure or tightness in the neck
- Difficulty swallowing or breathing (with large goitres)
- Hoarse voice that persists
- Signs of overactive thyroid: weight loss, fast heartbeat, tremor, heat intolerance, anxiety
- Many nodules cause no symptoms and are found on a scan done for another reason
Causes and risk factors
- Iodine deficiency (less common now with iodised salt, but still seen)
- Being female: thyroid problems are much more common in women
- Family history of thyroid disease
- Autoimmune conditions (Hashimoto's, Graves')
- Radiation to the neck in childhood (a risk factor for thyroid cancer)
When to see a doctor
See a doctor for any new neck lump. Seek prompt care if you notice:
- A lump that is growing quickly or feels hard and fixed
- Hoarseness lasting more than a few weeks
- Difficulty breathing or swallowing
- Enlarged lymph nodes in the neck
How thyroid swellings are assessed at Samrijo
- Examination of the neck and a history of symptoms.
- Thyroid function tests (TSH, T3, T4) and calcium in the in-house lab.
- Neck ultrasound: shows the size and features of nodules, which help decide which need a needle test.
- FNAC (fine needle aspiration cytology): a thin needle takes cells from a nodule, often under ultrasound guidance. Results are reported in categories (Bethesda) that guide treatment.
- Other tests as needed: thyroid scan, CT for large goitres, and a voice-box check before surgery.
Treatment options
Complex thyroid cancers may need a specialist team; we will guide you if that's the case.
What to expect
Before
Blood tests, ultrasound, FNAC, a voice-box check and an anaesthesia assessment. Overactive thyroid must be controlled before surgery.
During
Under general anaesthesia, through a cut in a neck crease. Surgery typically takes one to three hours, depending on the extent.
After
Mild neck discomfort and sore throat are common. Calcium levels are checked. Many patients go home in one to two days.
Follow-up
Stitch/wound check, tissue report, thyroid blood tests and a dose adjustment of thyroid tablets if needed. Recovery varies from person to person.
Life after thyroid surgery
- Voice: rest your voice for a few days if hoarse; it usually improves over weeks. Tell your surgeon if it doesn't.
- Calcium: report tingling around the lips, fingers or toes, or cramps, immediately.
- Thyroid tablets: take them every morning on an empty stomach, 30–60 minutes before tea or breakfast, and keep follow-up blood tests.
- Scar care: keep it clean; once healed, gentle massage and sun protection help it fade.
- Activity: most people return to desk work in one to two weeks. Gentle neck movements help stiffness.
Myths and facts
- "A thyroid lump means cancer." The large majority of thyroid nodules are benign. Tests tell which ones need attention.
- "Thyroid tablets will make my goitre disappear." Tablets correct hormone levels but usually don't shrink nodules or large goitres.
- "After thyroid surgery, I'll gain lots of weight." With the correct dose of thyroid hormone, metabolism is kept normal. Blood tests guide the dose.
- "An FNAC needle test spreads cancer." There is no good evidence for this. FNAC is a safe, standard test that often avoids unnecessary surgery.
- "Iodised salt is harmful." Iodised salt prevents iodine-deficiency goitre and is recommended for most people.
Questions to ask your surgeon
- What did the ultrasound and FNAC show, and what category is my nodule?
- Can we monitor it, or do you recommend surgery? Half or the whole gland?
- What is the chance I will need thyroid tablets afterwards?
- How will you protect my voice and calcium glands?
- When can I return to work, and when should I have my first blood tests?

Frequently asked questions
Does every thyroid swelling need surgery?
No. Many thyroid nodules and goitres are harmless and only need monitoring or medicines. Surgery is considered for nodules that are suspicious or cancerous on tests, large goitres causing pressure symptoms, or some cases of overactive thyroid.
Will I need thyroid tablets after surgery?
After total thyroidectomy, yes: you will need a daily thyroid hormone tablet for life, which is simple to take and monitored with blood tests. After removing half the gland (hemithyroidectomy), many people do not need tablets, but some do.
Can thyroid surgery affect my voice?
The nerves to the voice box run close to the thyroid. Temporary hoarseness is not uncommon and usually improves over weeks. Permanent voice change is uncommon. Surgeons take great care to identify and protect these nerves.
What is the calcium problem after thyroid surgery?
Four tiny parathyroid glands next to the thyroid control calcium. If they are stunned during surgery, calcium can drop temporarily, causing tingling around the lips or fingers. This is treated with calcium tablets, and is usually temporary.
How visible is the scar?
The cut is usually placed in a natural skin crease low in the neck, so it often fades well over months.
What does thyroid surgery cost?
It depends on the extent of surgery, tests (ultrasound, FNAC, blood tests), stay and insurance. Call 93196 62944 for an estimate.
