Liver abscess and liver surgery

The liver is a large organ on the right side of the upper belly. It filters blood, makes bile and stores energy. A liver abscess forms when infection creates a collection of pus inside it. There are two main types:

  • Amoebic liver abscess: caused by the parasite Entamoeba histolytica, spread through contaminated food or water. Common in India.
  • Pyogenic (bacterial) liver abscess: often linked to gallstones, bile duct blockage, diabetes or infection elsewhere in the abdomen.

This page also covers liver cysts and when liver surgery is needed.

Symptoms

  • Fever, often high, with chills or sweating
  • Pain in the upper right belly, sometimes spreading to the right shoulder
  • Pain on deep breathing or coughing
  • Loss of appetite, nausea, weakness and weight loss
  • Occasionally yellow eyes (jaundice) or diarrhoea

Causes and risk factors

  • Contaminated water or food (amoebic abscess)
  • Heavy alcohol use, which is linked to amoebic abscess in men
  • Diabetes and weak immunity
  • Gallstones or bile duct blockage
  • Infection elsewhere in the abdomen (for example, a burst appendix)
  • Recent procedures on the bile ducts

When to see a doctor

High fever with pain under the right ribs needs same-day assessment. A large abscess can burst if untreated. Call 93196 62944; we are open 24×7.

  • Sudden, severe belly pain or breathlessness
  • Yellow eyes, confusion or low blood pressure
  • Diabetes with fever and belly pain

How it is diagnosed at Samrijo

  • Examination and history, including travel, water source and alcohol.
  • Blood tests in the in-house laboratory: blood count, liver function, sugar, and tests for amoebic infection.
  • Ultrasound of the abdomen: the main test to find and measure an abscess.
  • CT scan for unclear or complex cases.
  • Aspirated pus may be sent for tests to guide antibiotics.

Treatment options

1. Medicines

All abscesses

Anti-amoebic medicines and/or antibiotics, usually for a few weeks. Many small amoebic abscesses heal with medicines alone.

Good for

  • No procedure needed in many cases

Keep in mind

  • Requires completing the full course and follow-up scans

2. Ultrasound-guided aspiration or drainage

Large / not improving

A thin needle, or a small tube (pigtail catheter), is placed through the skin into the abscess under ultrasound guidance, with local anaesthesia, to drain the pus.

Good for

  • Faster relief; avoids open surgery
  • Lowers the risk of rupture

Keep in mind

  • A drain may stay for several days
  • Sometimes needs repeating

3. Surgical drainage

Few cases

Laparoscopic or open surgery for a ruptured abscess, thick pus that cannot drain through a tube, or when another abdominal problem needs treatment at the same time.

Good for

  • Definitive drainage in complex cases

Keep in mind

  • Bigger procedure; longer stay

Liver cysts

Most simple liver cysts are fluid-filled, harmless, and found by chance on ultrasound. They need no treatment. Hydatid cysts, caused by a tapeworm usually spread by dogs, may need medicines and a procedure or surgery. Large cysts that cause pain or pressure may be treated laparoscopically. Imaging decides which type you have.

If a solid lump (rather than a cyst or abscess) is found in the liver, further scans and blood tests are usually needed to understand what it is. Many are harmless, such as haemangiomas. When specialised liver surgery or cancer care is needed, we will explain the findings clearly and help you reach an appropriate specialist centre without delay.

What to expect

Admission

Many patients with a liver abscess are admitted for a few days for IV medicines and monitoring.

Drainage

If needed, done under ultrasound guidance with local anaesthesia; it usually takes a short time.

Recovery

Fever usually settles within days of the right treatment. Tablets continue at home, and the abscess cavity shrinks over weeks.

Follow-up

Repeat ultrasound and blood tests. Drink safe (boiled or filtered) water and keep sugar under control to prevent recurrence.

Recovery at home

  • Finish every tablet in the course, even when the fever has gone. Stopping early can let the abscess return.
  • Drain care: if you go home with a drain, keep the bag below the level of the wound, note the amount and colour of fluid as shown, and keep the skin around it clean and dry.
  • Eat light, nourishing food: dal, khichdi, curd, fruits and plenty of safe fluids. Avoid alcohol completely.
  • Rest, then build up activity gradually. Most people return to light work within one to two weeks, but this varies.
  • Come back at once if fever returns, pain increases suddenly, you become breathless, or the drain falls out.

Preventing a liver abscess

Many amoebic liver abscesses begin with contaminated water or food. Simple habits lower the risk for the whole family:

  • Drink boiled, filtered or RO-purified water, especially during the monsoon.
  • Wash hands with soap before eating and after using the toilet.
  • Wash fruits and vegetables well; be careful with street food, cut fruit and chutneys left uncovered.
  • Treat dysentery (loose stools with blood or mucus) properly and completely.
  • Limit alcohol, and keep diabetes well controlled.

Questions to ask your doctor

  • Is my abscess amoebic or bacterial, and how does that change treatment?
  • Is it large enough to need drainage, or can medicines alone work?
  • How long will I need tablets, and when is the next ultrasound?
  • Which symptoms mean I should come back immediately?

Frequently asked questions

What is a liver abscess?

A liver abscess is a pocket of pus inside the liver. In India, it is most often caused by amoeba (from contaminated food or water) or by bacteria, sometimes linked to gallstones or diabetes.

Does every liver abscess need surgery?

No. Many liver abscesses heal with medicines alone, especially amoebic ones. Larger abscesses, or those at risk of bursting, are often drained with a thin needle or tube under ultrasound guidance. Open or laparoscopic surgery is needed only in a few cases.

How long does treatment take?

Medicines are usually given for a few weeks. If a drain is placed, it may stay for several days until the pus stops draining. Follow-up ultrasounds check that the cavity is shrinking. Recovery time varies.

Is a liver cyst dangerous?

Most simple liver cysts are harmless and need no treatment. Hydatid cysts (from a tapeworm) and large cysts causing symptoms may need treatment. Imaging helps tell the types apart.

Do you perform liver transplant or major liver resections?

Liver transplants are done only at specialised transplant centres. For complex liver tumours or major resections, we will guide you to an appropriate specialist centre if needed.

What does treatment cost?

It depends on whether you need medicines only, ultrasound-guided drainage or surgery, plus stay and tests. Call 93196 62944 for an estimate.
